Promed Hospital Saves Life of 90-Year-Old with Calcified Triple Vessel Heart Disease and severe LV Dysfunction by Performing Complex Multivessel PCI with world’s Smallest Heart Pump
Chennai, 10th January 2024: Dr. Arun Kalyanasundaram, an eminent Interventional Cardiologist in India and Chief Cardiologist at Promed Hospital , a leading multispecialty hospital at Kottivakkam, Chennai accomplished a remarkable feat by successfully performing a complex procedure of Intravascular Lithotripsy (IVL)-assisted Percutaneous Coronary Intervention (PCI) of Left Main (LM) to Left Anterior Descending (LAD) & RCA complemented by Impella CP Support and saved a life of a 90-year-old man with a complex cardiac history. It is to be noted that this is the oldest male patient successfully treated with this life-saving device in the country.
The patient a resident of Chennai was admitted at Promed Hospital with acute onset breathlessness, progressively worsened and he had chest pain. His ECHO showed severe LV dysfunction with a heart function of 20%. The Coronary Angiogram was performed on the patient which revealed that the patient had a critical Triple Vessel Disease and severely calcified coronaries in the heart. Patient had a history of Type II diabetes mellitus, systemic hypertension, and dyslipidemia. He was considered to be a very poor candidate for bypass surgery considering his age and the nature of his vessels and was turned down for bypass surgery. Hence, the decision to perform complex multivessel PCI in order to achieve optimal revascularization.
The patient underwent IntraVascular Lithotripsy (IVL)-assisted Percutaneous Coronary Intervention (PCI) a minimally invasive procedure and removed the blocked from coronary arteries. The LV pump ensured uninterrupted supply of blood to the patient’s body while this complex procedure was performed. The procedure took about 3 hours, patient was moved out of ICU the day following the procedure. He has since recovered and was discharged on the 4th day of the Procedure. The patient is doing well at 30 day follow up with no exertional symptoms and notably his heart function has improved to 45%.
Explaining about the case, Dr. Arun Kalyanasundaram, Interventional Cardiologist, Promed Hospital, Chennai, said, “We are extremely proud to have successfully performed the complex procedure of IVL-assisted PCI from the left main (LM) to the left anterior descending artery (LAD), and RCA with Impella CP support. This remarkable feat showcases the advanced capabilities of modern medical technology in providing innovative solutions for patients with complex cardiovascular conditions without the need for open heart surgery. It was a high-risk procedure as it involved an aged patient with complex cardiac history. It is to be noted that such a complex procedure had not been performed successfully in India before this. This procedure is definitely a breakthrough and showcases our expertise and technical advancements offered by our hospital.”
The surgery, performed at Promed Hospital, highlighted Dr. Arun Kalyanasundaram’s creative approach to treating complex coronary artery diseases. During this groundbreaking case, Impella CP Support, a cutting-edge gadget, provided cardiac support and ensured excellent hemodynamic stability throughout the procedures. By integrating this technology, Dr. Arun Kalyanasundaram exemplified his commitment to employing the latest advancements in medical devices to enhance patient outcomes and safety, said, Dr Spoorthi Arun, MD, Promed Hospital
